While an unsupportive pillow often gets the blame, neck pain at night is rarely caused by one thing alone — sleeping position, daytime posture, muscle tension, and underlying neck conditions can all play a role. Why Does My Neck Hurt at Night?

Your sleeping position

Sleeping with your head turned, tilted or unsupported for several hours can place additional strain on the muscles and joints in your neck. Stomach sleeping commonly requires the neck to remain rotated to one side, while side sleeping with a pillow that is too high or too low can leave the neck bent throughout the night.

Ideally, your pillow should support your head while keeping your neck in a comfortable, neutral position. The correct pillow height will depend on your body shape, mattress and preferred sleeping position.

Daytime posture and prolonged positions

Your nighttime symptoms may actually begin during the day. Long periods spent looking down at a phone, working at a computer, driving or sitting with your shoulders rounded can increase strain on the neck and upper-back muscles. Symptoms may become more noticeable at night when you stop moving and settle into one position.

Muscle tension and stress

Stress can contribute to increased tension around the neck, shoulders and jaw. Some people also clench or grind their teeth during sleep, which may aggravate the muscles around the jaw and upper neck.

Reduced movement during sleep

Movement helps prevent joints and muscles from becoming stiff. During sleep, your neck may remain in one position for an extended period. An irritated joint, sensitive muscle or underlying condition may therefore feel more uncomfortable overnight or first thing in the morning.

Although nighttime neck pain is commonly caused by sleeping position & the above-discussed, persistent unrelenting pain—particularly when accompanied by fever, unexplained weight loss, neurological changes, significant trauma or a history of cancer—should be assessed promptly by a medical professional.

How Can You Help with Neck Pain at Night?

Start by checking your sleeping position. Your pillow should fill the space between your head and mattress without pushing your head too far forward or allowing it to drop sideways.

You can also try:

  • Applying gentle heat to the neck before bed.
  • Performing comfortable neck movements rather than keeping the neck completely still.
  • Taking regular movement breaks during computer or phone use.
  • Adjust your workstation so your screen is closer to eye level.
  • Using relaxation or breathing exercises before sleep.
  • Avoiding forceful stretches or repeatedly “cracking” your neck.
